In this presentation, Susan will be discussing "Facilitating the Root Cause Analysis Approach: Changing the Face of Patient Safety." Root Cause Analysis is a methodically applied way to identify and correct the root causes of events, rather than to simply address the symptomatic result. To be effective, root cause analysis must be performed systematically, usually as part of an investigation, with conclusions and root causes that are identified backed up by documented evidence. A team effort is typically required.


Susan will be taking us through the information gathering process conducted as a critical part of a successful Root Cause Analysis, offering practical advice on how to conduct this effort efficiently and thoroughly, leading to a successful outcome.

Our Presenter 


Susan L. Cresswell is the Director of Integrated Quality Services at Sutter Roseville Medical Center. After receiving her Master’s Degree in Health Care Administration from Trinity University, Susan has worked in health care administration for hospitals in Tucson, Arizona, Tyler, Texas, and Medford, Oregon, prior to her current position.  Susan is certified as a Professional in Healthcare Quality and as a Healthcare Environmental Manager.
